Following is the Intake Checklist for Entrance into Philadelphia Teen Challenge. Each requirement is very important. They are as follows:

• Each Applicant must call to schedule a face-to-face interview.
  
  
• Note: If client is incarcerated and unable to attend interview with the Intake Supervisor, then two (2) reference letters from authorities (probation / parole officers, chaplains, etc.) can be forwarded on behalf, plus an over the phone interview.
  
  
• Each Applicant must submit a Physical Examination.
  
  
• Each Applicant must submit Social Security Card, Birth Certificate and official photo ID.
  
  
• Each Applicant must submit a one-time, non-refundable Intake Fee of $750.00 plus return fare.
  
  
• Each Applicant must submit a completed Sponsor's Form.
  
  
• Philadelphia Women's Home requires further lab work.
  
  
• It should be understood that a "Bed Date" will NOT be assigned until the Intake folder is complete! Thank you for applying to Teen Challenge of Philadelphia.
